Hospice is dedicated care for the specialized needs patients face as they approach end-of-life.

A Promise of Hope Hospice & Palliative Care is here to help our patients and their loved ones through a very difficult time. We offer hope to our patients and their caregivers by assisting them through the transition from curative medicine to comfort care as they approach end-of-life. Our number one goal will always be patient-centered comfort and pain management, delivered at home by our caring, compassionate team of licensed professionals. This process eases the financial and emotional burdens on the family, while ensuring the patient is treated with dignity and respect and their final wishes are honored.

Our Team of Caring Professionals

Our Interdisciplinary Team (IDT) is led by medical director, Dr. Gary Tubbs. The team consists of the patient’s Primary Care Physician, Registered Nurses, Certified Nursing Assistants, Chaplains, Social Workers and Volunteers.

Our patients can receive care in any of the seven southwestern Idaho counties we currently serve:

• Canyon
• Elmore
• Gem
• Owyhee
• Payette

As individuals are faced with the diagnosis of a terminal illness, the majority would choose to live the remaining days of their life in the comfort of their own home. All too often, this does not happen simply because people are not aware of the availability of hospice, the cost savings it can bring, as well as the ease of becoming involved with hospice and the support it provides for the patient, their loved ones and/or their caregivers
